What are the Different Levels of Trauma Centers?

As a Sacramento Personal Injury Lawyer, over the years I’ve been asked what the difference is between the different types of trauma centers. There are different levels of trauma centers: Level I, II, III, IV, and V. These refer to the types and kinds of resources available at the facility and the number of trauma patients admitted each year. The categorization is different for adult and pediatric facilities. Category requirements are different from state to state and are outlined by legislation. Verification of a trauma center’s level is set through the American College of Surgeons and is designed to improve overall trauma care. Verification lasts for three years and then is reevaluated.Level I Trauma Center

Because a trauma level is different for children and adults, a facility can be considered Level I for adults and Level II for children. A level I trauma center has 24-hour in-house coverage by specialists in surgery as well as the prompt availability of neurosurgeons, orthopedic surgeons, anesthesiologists, emergency medical specialists, internal medicine specialists, pediatricians, plastic surgeons, radiologists and maxillofacial surgeons. It stands as a referral source for nearby hospitals and ambulances. It is a leader in public education and prevention of trauma and provides continuing education for its trauma team members. It has a vast quality assessment program. It has a program for substance abuse and has an organized research and teaching program. It sees many trauma patients each year. If you’ve been seriously injured in an automobile accident or other traumatic incidents, it is a good idea to have your injury treated in a level I trauma center. This is a medical facility that has a proven record of being able to treat trauma of the highest degree.Level II Trauma Center

A level II trauma center is able to treat most injured patients. It has 24 hour instant coverage of all medical specialties associated with trauma, including critical care coverage. Certain things like microvascular surgery, heart surgery, and hemodialysis are usually referred to a level I center. A level II facility has a comprehensive quality assessment program and provides education to its staff members.Level III Trauma Center

A level III facility has 24-hour coverage by emergency medicine specialists and immediate availability of general surgeons and anesthesiologists. It has a comprehensive quality control program. It has transportation and transport agreements with level I trauma facilities if a patient is unstable. It provides back up for area rural and community hospitals. It provides continuing education to the trauma team and must have an active outreach program for its referring communities.Level IV Trauma Center

A level IV trauma center can provide advanced trauma life support or ATLS before sending the patient to a higher level trauma center. It has 24-hour lab services and trauma nurses and doctors available at all times. It may or may not provide surgical services or critical care services. It has transfer agreements to higher level facilities and has an active quality control program. It has an active outreach program for its referring communities.Level V Trauma Center

A level V trauma center is able to give an initial evaluation and stabilization of a  patient in order to prepare them for higher care levels. It has a basic emergency department for ATLS protocols and has a trauma nurse and doctor available at all times. It has after-hours protocols if not open all the time and it might have surgical and critical care services. It has transfer agreements with higher level facilities.Sacramento Trauma Attorneys

A trauma center is an area of a hospital that’s equipped to treat the most high-risk of injuries — think gunshot wounds, serious car crash injuries, and major burns.

If you experience physical trauma, you’ll receive treatment through a hospital’s emergency department — but not all emergency departments are created equal. In addition to providing emergency care, some hospitals also function as trauma centers, and they’re staffed by specially trained health professionals who treat life-threatening injuries.

Trauma centers offer more extensive care than emergency departments, and the difference between a trauma center and emergency room can be life and death. So, it’s important to understand what these facilities offer.The Difference Between Trauma Centers and Emergency Rooms

Trauma centers are typically located within hospitals, often in the emergency department. Emergency rooms provide care to people suffering injuries ranging from a sprained ankle to a heart attack — and they are staffed with doctors, nurses, and medical experts who handle a variety of conditions.

They staff the center 24/7 and have access to resources such as an operating room, resuscitation area, laboratory, and diagnostic testing equipment. They are always prepared to treat patients.When should you go to a trauma center?

There are five different levels of trauma centers in the United States, but they can vary from state to state, and not every state recognizes all five levels. Pennsylvania only recognizes Levels I through IV, for example.

The American Trauma Society describes the five levels as:Level I: The center provides total care, from prevention through rehabilitation. These also offer a teaching program for medical residents, as well as ongoing research.Level II: The center is similar to a Level I trauma center but doesn’t necessarily offer teaching or research. Both Levels I and II can treat either children or adults.Level III: The center is smaller than Level I and II centers but can provide prompt care to injured patients.Level IV: The center can provide trauma care and life support before patients are transferred to a larger, higher-level trauma center.How are trauma centers accredited?

All trauma centers must be accredited (officially recognized) by an independent organization. This accreditation varies by state. In Pennsylvania, the Pennsylvania Trauma Systems Foundation has overseen the accreditation of hospitals since 1986.What Is a Trauma System?

A trauma system includes many different facilities and services that work together to provide a full range of care for those who are severely hurt. For example, a trauma system might include a Level I trauma center, emergency medical services, rehabilitation centers, and prevention organizations. UPMC Mercy and UPMC Presbyterian are both Level I trauma centers, while UPMC Hamot and UPMC Altoona are Level II. UPMC Children’s Hospital of Pittsburgh serves as a Level I pediatric trauma center. Trauma centers at UPMC are equipped to treat the most seriously injured patients, with operating rooms and specially trained doctors and nurses ready at all times.

UPMC has written the book on trauma care — literally.  Andrew Peitzman, M.D., of UPMC published “The Trauma Manual” 1998. The book, currently available in its 4th edition, is the first comprehensive guide to trauma center care. The manual includes sections on the specifics of patient injury and resuscitation, as well as common problems that trauma staff may encounter with a high-risk patient.

Trauma center merupakan fasilitas yang disediakan oleh pusat layanan kesehatan guna menangani pasien khusus yang mengalami trauma/kecelakaan kerja. Layanan ini melibatkan banyak dokter ahli dalam penanganannya, misalnya dokter bedah, dokter anestesi serta perawat-perawat lainnya. Pelayanan pada trauma center ini biasanya siap melayani 24 jam dengan fasilitas lengkap seperti Instalasi Rawat Darurat (IRD), ambulance, laboratorium, radiologi, kamar operasi, Intensive Care Unit (ICU), rehabilitasi hingga apotek.

Tipe-tipe layanan trauma ada beberapa macam, di antaranya:

Banyak sekali trauma yang akan dialami berbagai jenis pasien, seperti trauma akibat kecelakaan kerja, trauma terhadap lingkungan kerja yang kurang memadai, atau penyakit akibat kerja. Namun, ada juga beberapa trauma lainnya yang dapat ditangani seperti trauma akibat masalah keluarga, pertemanan, percintaan dan sebagainya. Jadi tipe trauma dapat dibedakan berdasarkan trauma fisik dan psikis.

Trauma center berdasarkan rumah sakit atau klinik yang dituju. Ada beberapa rumah sakit yang menyediakan trauma center secara mandiri dan dengan adanya penambahan pelayanan sesuai dengan kebutuhan pasien. Biasanya layanan ini mematok harga tertentu yang tentunya beda dengan harga yang telah ditetapkan pemerintah yaitu dengan menggunakan layanan BPJS.

Level/Tingkatan Pada Trauma Center

Selain tipe-tipe trauma, ada juga level atau tingkatan dari trauma itu sendiri. Level atau tingkat trauma itu dibagi menjadi 5, yaitu:

Kasus yang ditangani pada trauma level I ini adalah kasus trauma secara menyeluruh, yang diambil dari sumber daya daerah setempat, sarana pendidikan, penelitian dan perencanaan dari sistem. Biasanya akan ditangani oleh tenaga ahli seperti dokter bedah trauma, dokter bedah umum, dokter anestesi, perawat dan segala perlengkapan resusitasi.

Kasus yang ditangani pada trauma level II adalah kasus secara menyeluruh pada daerah perkotaan dan rumah sakit utama dengan kepadatan penduduk yang lebih tinggi. Pada daerah perkotaan ini tidak diperlukannya sarana pendidikan ataupun penelitian yang berkelanjutan.

Kasus yang ditangani pada trauma level III ini adalah kasus tanpa adanya pelayanan dari ahli atau spesialis secara penuh, namun memiliki sumber daya yang cukup seperti ruang gawat darurat, beda, dan pelayanan intensif. Pelayanan kasus pada level III ini lebih menyeluruh pada masyarakat luas baik pada level I dan level II.

Kasus yang ditangani pada trauma level IV dan V ini lebih berfokus pada resusitasi dan stabilisasi pasien dengan mengatur proses rujukan pasien trauma. Proses rujukan ini dilakukan supaya pasien mendapatkan perlengkapan dan lokasi yang lebih lengkap dan strategis.

Penanganan yang cepat pada korban kecelakaan diperlukan dengan melibatkan dokter ahli.

Nah, dari masalah trauma di atas maka dari itu saat ini telah banyak rumah sakit dan klinik di Indonesia yang menyediakan fasilitas keselamtan kerja ini. Fasilitas tersebut tidak sembarang rumah sakit bisa memilikinya, hanya pada rumah sakit dan klinik tertentu saja yang telah memenuhi syarat dan prosedur yang ditetapkan.
